I believe there must be a simple answer to this problem, but d**n if I can figure it out. My passenger side arm rest was falling off, only being held on in one of the three places (two on the bottom, one at the top) it should have been. At the bottom, one of the clips is missing, so I'll have to find one. No problem. So, I removed it and the door panel in order to fix it. The trouble is with the top of the thing. From what I can see, it has to be attatched, by a screw, which goes through a metal plate which has to be positioned behind the inside metal of the door, then into the top of the arm rest, where the chrome trim is. And I can not even imagine how this is done, given that it has to be done after the door panel is in place. Anyone know how this is done?

Hmmm
I haven't had the door panels of for awhile but I don't think I had to remove the top connection to remove the door panel from the door on my '76 but its been awhile.
